Great Dane Knowledge: What is your experience with the Great Dane breed? Are you familiar with their unique health needs (Bloat, DCM, Pano, etc.) and average lifespan?

Have you owned a dog before? If yes, please list the breeds, and explain what happened to the dog(s).

• Current Pets: Please list all current pets in the household (species, breed, age, and spay/neuter status).

Household Members: Please list all adults and the ages of any children living in the home.

Do you have a professional dog trainer or training facility already selected for your puppy's basic obedience and foundational socialization classes? (Please provide the trainer/facility name and contact information if Yes.)

What kind of training methods do you plan to use? (Note: Duty Danes strongly encourages force-free, positive reinforcement methods.)

Service Dog Training: If you intend to train the puppy as a Service Dog, do you have an owner-trainer mentor or a program secured to guide the specialized task training?

Residence: Do you own or rent your home?

Landlord/HOA: If you rent, please provide the name and phone number of your landlord or HOA contact who can confirm a Great Dane is permitted.

Yard Security: Do you have a fenced-in yard? If so, what type and height?

Primary Caregiver: Who in the household will be the primary person responsible for the dog's daily feeding, grooming, and emotional well-being?

Separation: How many hours per day, on average, will the dog be left alone? Where will the dog be kept when you are away from home?

Common Challenges: How will you handle standard puppy and adolescent Great Dane challenges, such as destructive chewing, house-training accidents, and high-energy "zoomies"?

Service Dog Intent: Do you intend to train this puppy as a Service Dog? (Yes/No/Unsure/Therapy Dog/ESA)

Specific Task: If Yes, please describe the specific work or task the dog will be individually trained to perform to mitigate your disability.

• (Example: "Deep Pressure Therapy (DPT)," "alerting to rising anxiety levels," "Mobility Assistance for bracing," etc.)

Duty Danes Breeder Note on Service Dog Placement: We ask about the specific task not to question the legitimacy of a disability, but because our mission is "Healing Hearts one paw at a time." This is about ensuring we select the most appropriate puppy from our litter, matching its size, structure, and temperament to the required task to set the puppy up for success.

If the puppy is being acquired primarily for the benefit of a child (under 18):

Primary Adult Caregiver: Who is the primary adult caregiver in the home responsible for the dog’s daily feeding, grooming, and veterinary care?

Primary Handler: Who will be the primary handler responsible for the dog’s training, control, and supervision, particularly during public access outings?

• Transition Plan: At what age do you anticipate the child beginning to take over full handling responsibilities for the dog, and what training steps will you put in place for that transition?

Expectations: What did you love most about your last or current dog? What did you like least?

Lifestyle and Household

Household Activity Level: Describe the activity level of your household (e.g., quiet, active, hectic).

Time Alone: How many hours will the puppy be left alone on a typical day? Where will the puppy be kept during this time?

Sleeping Arrangements: Where will the puppy sleep at night?

Primary Caregiver: Who will be the primary person responsible for the puppy's care and training?

Sex Preference: Male, Female, or No Preference.

Color/Pattern Preference: (e.g., Harlequin, Mantle, Merle, Blue, Black).

Role of Dog: Is this dog intended to be a Family Pet, Service Dog prospect, or Therapy Dog prospect?

Emergency Fund: Are you prepared for the financial responsibility of a Great Dane, including high-quality food, large-breed vet care, and emergency expenses (e.g., Bloat surgery)?

In order for your $300.00 non-refundable reservation list fee to be accepted, please agree to the following terms:

I understand and agree that my $300.00 Retainer fee is non-refundable. I am not guaranteed my puppy of choice or gender of choice; however, I understand that I can continue to defer my place in line and add myself to future reservation lists until I get the puppy I want and /or need.

I understand and agree to the terms and conditions

I Agree

I also understand that the timing of litters is uncontrolled, as well as a successful pregnancy. We are not able to control the timing of heat cycles. We also have the right to cancel a reservation due to the best interest of the parents involved (the health of our parent dogs is paramount to having a litter). In the event a reservation list (breeding pair) is cancelled, I will be given other upcoming options. Heat cycles and timing is not guaranteed.

_ I understand and agree that timing is never guaranteed

. I understand that clients choose in the order of the reservation list and clients can choose any of the puppies available to them at their selection time and placement. If a client is not in attendance nor is reachable by phone during their 30-minute selection timeframe, then they will forfeit their placement.

_ I understand and agree to the selection day process.

When clients pay a non-refundable retainer fee they are allowed to be on two reservation lists concurrently and ONLY choose ONE puppy. If I want two puppies, I understand that two retainer fees are required. Once I select my puppy, I will be removed from the other reservation list. I understand that I cannot use the second spot for a friend or family member.

I understand that my retainer fee places me on two reservation lists; however, that is only for one puppy. When I choose my puppy, my name will be removed from the other list. It cannot be transferred to someone else.

We reserve the right to use any puppy selections in any form or fashion without restriction. We also reserve the right to replace a client that chooses to remove them self from the list with our own selection.

I understand and agree to Duty Danes’ right to use their pick in any form or fashion they choose.

I am aware that puppies can have certain common ailments upon coming home. Urinary tract infections, parasites, ear infections and allergy issues are all common. While we routinely deworm and monitor for potential UTIs and ear infections, sometimes the stress of leaving the breeder causes issues to flare up. I understand it will be my responsibility to treat and care for my new puppy once they leave the breeder.

_ I understand that puppies can have certain ailments and I agree to ensure they get the proper care and medication, if needed.

I understand that I can be on two lists concurrently, unless both lists have pups in the nursery at the same time. understand that due to safety concerns, we no longer ship puppies. You must pick up your puppy or pay for a nanny transport.

I agree to choose which list once both litters have had puppies and agree to be responsible for all costs associated with picking up the puppy on selection day (either myself or a puppy nanny.

By signing your name below, you agree to Duty Danes' terms and conditions regarding the non-refundable reservation fee.
